ABSTRACT

La tuberculosis persiste siendo un problema importante a nivel mundial. A pesar de que el compromiso de la cadera es infrecuente, su aparición representa un escenario complejo. Presentamos un paciente masculino que desarrolló una infección de la cadera que fue diagnosticada como tuberculosis de forma tardía. El paciente recibió tratamiento médico por 6 meses y luego se realizó una artroplastia total de cadera en dos tiempos, con éxito. Presentamos el reporte de un caso completo, junto a su seguimiento al aão y una revisión de la literatura sobre el tema.


Tuberculosis remains a significant problem worldwide. Even thought hip involvement is an infrequent manifestation, it́s appearance represents a challenging scenario. We present a male patient who developed a hip infection later discovered to be tuberculosis. The patient received a medical treatment protocol for 6 months and was subsequently treated with a two stage total hip arthroplasty. We present the full case report with it́s 1-year outcome and a literature review.

ABSTRACT

La Tuberculosis (TBC) es una patología infecto-contagiosa de alta morbimortalidad en Chile y en el mundo, siendo la segunda causa de muerte por cuestión infecciosa y es considerada una patología de alta relevancia a nivel de salud pública. Es causada por una bacteria de alta virulencia y contagio llamada mycobacterium tuberculosis. En la actualidad contamos con protocolos de detección y tratamiento muy eficaces, que la convierten en una enfermedad prevenible y curable. El diagnóstico se realiza con estudios bacteriológicos específicos frente a una sospecha clínica-epidemiológica sugerente. Sin embargo, el uso de imágenes forma parte casi obligatoria de su estudio y control. Debido a que el órgano diana de la TBC es el pulmón, es habitual utilizar como apoyo diagnóstico una radiografía de tórax, la cual es útil, en caso de TBC pulmonar, al presentar hallazgos característicos y orientadores para su diagnóstico. Es importante destacar que el mycobacterium tuberculosis tiene alto potencial de diseminación por contigüidad, vía linfática y/o hematógena, siendo esa última vía la causante de la mayoría de las TBC extrapulmonares, las cuales se presentan en un 20% de pacientes inmunocompetentes y hasta en un 60% de inmunocomprometidos. La principal localización de una TBC extrapulmonar es a nivel pleural, seguida del compromiso ganglionar, urogenital y osteoarticular, siendo el resto de las localizaciones muy infrecuentes. Para esos casos la tomografía computada (TC) es el estudio por imágenes de elección para el diagnóstico y control, además de ser una herramienta muy útil para la detección de complicaciones.


Tuberculosis (TB) is an infectious disease of high morbility and mortality in Chile and in the world. It is the second cause of death due to infectious causes in the world, and is considered of high relevance to public health. TB is caused by a highly pathogenic and virulent bacterium denominated mycobacterium tuberculosis. Nowadays, there are effective protocols for detection and treatment of this disease, which make it preventable and curable. Diagnosis is reached by specific bacteriological studies in the presence of a clinical epidemiological suspicion. Nevertheless, imagining methods are almost an obligatory part of tuberculosis study and control. Since the lung is the target organ of TB, chest X-ray is commonly used as a support for diagnosis, which is very useful in case of pulmonary TB because it provides characteristic findings to guide diagnosis. It is important to highlight that the mycobacterium tuberculosis has a high potential for dissemination by contiguity, via lymphatic and/or haematogenous, the latter being the cause of the majority of extrapulmonary TB, which are presented in 20% of immunocompetent patients and by up to 60% of immunocompromised. The main site of extrapulmonary TB is into the pleural space, followed by the lymph node, urogenital and osteoarticular involvement, the remainder being infrequent localizations. In these cases, a computed tomography (CT) study based on the selection of images, is the tool used for diagnosis and control, which is also useful for the detection of complications.

ABSTRACT

Resumen La tuberculosis monoarticular aislada de la muñeca es una forma infrecuente de presentación de esta enfermedad, siendo más común el compromiso vertebral. Las formas extravertebrales representan sólo 2 a 3% de las infecciones óseas por Mycobacterium tuberculosis. Presentamos el caso clínico de una mujer de 49 años, con antecedentes de trabajar en labores de aseo en un hospital, que posterior a un trauma de baja energía evolucionó con un cuadro de dolor en la articulación de la muñeca derecha. Diagnosticada inicialmente como una tendinopatía flexora, recibió tratamiento con antiinflamatorios y fisioterapia. Ocho meses después la paciente continuó con dolor a la movilización por lo que se realizó un estudio imagenológico, biopsia y cultivos de tejido óseo. El estudio histopatológico y de biología molecular del tejido confirmó una tuberculosis de muñeca derecha. Se trató con terapia anti-tuberculosa y fisioterapia, consiguiéndose la recuperación funcional de la muñeca.


Monoarticular tuberculosis of the wrist is a rare presentation of primary tuberculosis, being more common skeletal forms involving the spine. Extraspinal tuberculous osteomyelitis is rare and comprises only 2 to 3% of all cases of osteoarticular Mycobacterium tuberculosis infections. We present a case of a 49 years old female patient, who worked as an hospital cleaning employed without other comorbidity. After a low energy injury of the wrist she suffered pain syndrome diagnosticated as a flexor tendinopathy, managed with nonsteroidal antiinflammatory drugs and physical therapy. Eight months later patient evolves with chronic pain in range of motion of right wrist joint, leading to a complete radiological, surgical biopsy and cultures. Histology, and molecular biology confirmed the wrist joint tuberculosis diagnosis. Pharmacological treatment and physical therapy were initiated with appropriated response.

ABSTRACT

We presented three patients with trochanteric tuberculosis and described the clinical and imaging findings of the infection. Histology revealed a necrotizing granulomatous bursitis and microbiology confirmed tuberculosis. All cases were successfully treated with bursectomy and curettage of the trochanteric lesion and antituberculous chemotherapy including isoniazid, pyrazinamide, rifampicin, and ethambutol.

Recently, there has been an epidemic of tuberculosis, with increasing incidence of infected persons in UK. In the past 20 years, with the pandemic of the human immunodeficiency virus and the increase in immunocompromised people, we have observed a resurgence of tuberculosis. Tuberculosis arthritis of the elbow joint is not commonly seen by orthopaedic surgeons. Spinal and monoarthritis of weight-bearing joints such as the hip or knee are most frequently involved. The aim of this case report is to describe a rare case of occult tuberculous arthritis of the elbow in young Indonesian lady and the diagnostic problems that may arise and lead to a delay in treatmen. It is a case report where we used different laboratory test as CBC, ESR, CRP, hepatitis screening and AIDS Antibody test to help in diagnosis.Plain X-ray for elbow and chest used. CT scan for elbow, aspiration of synovial fluid for culture and synovial biopsy taken. Tuberculous arthritis remains low on the index of suspicion of many clinicians working outside the field of respiratory or infection diseases3 Involvement of the skeletal system is relatively rare. Musculoskeletal TB is responsible for between 1.3% and 5% of all cases of TB. Osteoarticular TB should be suspected in patients of south Asian and African origin presenting with bony and soft tissue infective lesions

ABSTRACT

Tuberculous dactylis is uncommun which is difficult to differentiate from tumorous and metabolic affections. The histological studie constitutes a reliable and easy element to achieve for the positive diagnosis especially as the biologic studie can be negative. We report the case of a 43 year old woman who consulted for a post traumatic painful inch of the right hand. The x-ray of the hand visualized a fracture of the first phalanx of the inch with a lyse bony. Pathology examination of a biopsy specimen reveled granulomatous with caseous necrosis specific of tuberculous. The clinical signs and radiographs resolved after anti tuberculosis treatment for 12 months

ABSTRACT

Tuberculosis involving sacro-iliac joint and pubic bone presenting with massive retroperitoneal abscess is a rare entity. A 29-year-old female presented with history of discharging sinus in the sacrococcygeal region of 2 months duration. Plain x-ray revealed osteolytic lesion in right pubic bone and left sacro-iliac joint. Computed tomography scan revealed massive pus collection in the retroperitoneal region. Pus was drained extraperitoneally. Biopsy of the scraping of the abscess wall showed granulation tissue with foreign body type of giant cell. On follow-up the patient was doing well.

ABSTRACT

Total hip arthroplasty was performed in a patient who had tuberculous hip, quiescent for the last 15 years, without pre or postoperative anti-tuberculous chemotherapy. At a 27 month follow up, there has been no evidence of reactivation. A brief review of relevant literature is also presented.
